The Community Health Worker will be responsible for working with individuals that receive Michigan Medicaid benefits through UnitedHealthcare Community Plan.

Primary Responsibilities:

  • Member outreach
  • Member education
  • Assess if eligible for MiBridges and SNAP and assist with application
  • Partner with care team (community, providers, internal staff)
  • Knowledge and continued learning of community cultures and values
  • May conduct Health Risk Assessments (HRA) if needed
  • Assisting Healthy Michigan Plan (HMP) members with reporting work requirements
  • Work to increase dental utilization for all HMP members
  • Assess if eligible for MiBridges and SNAP and assist with application
  • Work to support new Mandatory Dual Special Needs Plan Reasonable Assistance requirements
  • Assist in achievement of health plan quality metrics such as: Postpartum and newborn education reinforcement for enrollment in the HFS/Baby Blocks program to keep members on target for well exams and shot schedule for 0-24 months, support low birth weight reduction by connecting members to SDOH resources in a face to face capacity (an enhancement to current national HFS model which is telephonic) such as prenatal classes, transportation, housing, food and social support and assessing for other risks, work to help support HEDIS metrics that impact auto band assignment
  • Work with members in addressing Social Determinants of Health
  • Build relationships with Community Based Organizations (CBO) as well as key Providers and Specialists
  • Help member set goals and develop a care plan to achieve those goals
  • Proactively engage the member to manage their own health and healthcare
  • Provide member education on community resources and benefits
  • Conduct post discharge activities from hospital facility and support connection to social services
  • May conduct Health Risk Assessments (HRA) if needed

Required Qualifications:
  • High School Diploma / GED (or higher)
  • Experience with Michigan Medicaid policies and procedures
  • Knowledge of culture and values in the local community
  • Familiarity with the resources available within the community
  • Candidate must be willing to travel up to 75% between the home office and the field
  • Must have access to reliable transportation that will enable you to travel to member and/or member sites within a service delivery area
  • Candidate must have the ability to create, edit, copy, send and save documents, and correspondence, using Microsoft Word, and Microsoft Outlook

Preferred Qualifications:

  • Bachelor's Degree (or higher) in social work and / or health care administration
  • Previous experience with Michigan Medicaid policies and procedures
  • Certified Nursing Assistant, Home Health Aide or Medical Assistant
  • Bilingual in Spanish and English
  • Experience working in Managed Care
  • MiCHWA Certification
  • Previous experience working with HMP / Medicaid population
  • A bility to create, edit, copy, send and save spreadsheets using Microsoft Excel
