Program Information: The Illinois Department on Aging has contracted HHCS to provide housing placement for individuals exiting nursing homes, under the Colbert Consent Decree. Managed Care Entities will provide referrals to HHCS, and subsequently HHCS staff will place the individuals into scattered-site apartments, a Supportive Living Facility, or other housing options, as appropriate. Referrals may originate anywhere throughout the city and suburban Cook County. HHCS staff will be involved with the coordination of other involved entities, from the point of referral, through housing placement, including any referrals for Assistive Technology (if needed for home modification), and assistance with buying household items. Developing relationships with the Managed Care Entities, Housing Authority of Cook County, and other partners of Colbert will be essential.

Summary: This position provides functional/operational supervision to the Housing Resource Specialists through progressively more complex and responsible levels. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below. This position will also provide some direct service work and housing placement with Program Participants.

Essential Duties and Responsibilities:

  • Oversees all participant assignments to Housing Resource Specialists in accord with established organization and department policies/procedures. Monitors staff attainment of project goals and prepares internal and external progress reports, accurately and on time.
  • Creates plan with Director to ensure all program scopes are met or exceeded.
  • Develops relationships with landlords to increase housing portfolio of affordable units, as well as relationships with Supportive Housing Facilities, and other housing entities as needed.
  • Assesses and ensures quality of housing inspections completed by Housing Resource Specialists.
  • Supervises the case plan, referrals, follow-up schedule and advocacy needs, etc. for participants/cases of the program.
  • Participates in the development of long range plans, service delivery model and other significant strategies and/or systems as requested.
  • Assumes responsibility for the accuracy, substance and timeliness of program reports, projects, schedules and other relevant work products.
  • Participates in interviewing, hiring, discipline, termination and other relevant employment transactions, with guidance from higher level Manager(s).
  • Provide regular supervision to assigned staff, regular communication, and professional development and guidance. Work in accordance with the Collective Bargaining Agreement and established Policies and Procedures.
  • Prepares performance evaluations for assigned staff, with guidance from higher level Manager(s).
  • Ensures the implementation of organization and program/department policies and procedures. (e.g. personnel, financial, participant eligibility, Standards of Professional Conduct, Ethics, HIPAA etc.)
  • Develops and maintains professional relationships with appropriate groups/organizations, etc., as approved by higher level Management.
  • Remains current in the field of service provided by the program through self-initiated avenues, as approved by higher level Management.
  • Provides training and guidance to assigned staff and ensures documentation is completed in a thorough, accurate and timely fashion.
  • Conducts organization-sponsored in-service training sessions, as requested.
  • Initiates requests for regular supervision and interim evaluation/goal setting.
  • Participates in budget development and/or grant writing for the program as requested.
  • Approves non-personnel financial transactions within established parameters.
  • Maintains an appropriate professional demeanor toward co-workers, visitors, participants, supervisors and all others.
  • Assumes an appropriate role in other program or Division tasks or projects, as assigned.
  • Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), and private funding sources. Assists in the development of organizational plans/policies related to professional standards. Provides periodic evaluative reviews and/or in-house and external staff training to ensure that staff understand and continue to adhere to such standards. Initiates requests for assistance or guidance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.

Other Duties and Responsibilities

  • The Supervisor of this position is also responsible for carrying a caseload. As such, the Supervisor will fulfill duties of a Housing Resource Specialist and place individuals from nursing homes into housing, as outlined in Colbert scopes.

  • Education and/or Experience:

Bachelors Degree in a Human Services related field and two years of relevant experience is required. May give credit towards years of experience if attained a degree higher than a Bachelor's Degree. One year of management, supervisory, or demonstrated progressive leadership experience is preferred. Additional experience in community-based work with one or more of the following: criminal justice involved populations, substance users, people with HIV/AIDS, mental illness, homelessness history, and/or related populations is preferred.

  • Other Skills and Qualifications: A valid Illinois driver's license is required and access to personal vehicle to use for work related business is required.
  • Language Skills: Excellent oral and written communication skills. Demonstrated competency in public speaking and strong interpersonal skills required.
  • Organization Skills: Excellent organization skills are required, including tracking many different pieces of information accurately.
  • Computer Skills: Proficiency in Microsoft Office, including Excel; Internet Explorer; Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance. Proficiency in online databases including HMIS.
