Summary: The Supervisor of FamilyWorks Wellness Team is responsible for functional and operational oversight of clinical/wellness services inclusive of administrative supervision and service delivery management, quality management and program development/performance outcomes. The supervisor ensures the successful provision of clinical services to participants aligned with Heartland Alliance's Philosophy of Care; attains FamilyWorks program outcomes per agency and funder requirements. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.

Essential Duties and Responsibilities:

  • Provides administrative oversight, including, but not limited to, participant/case/project assignments, staff employment transactions, implementation of policies and procedures, staff training, etc. in accordance with HHCS core competencies and guidance for FamilyWorks operations
  • Provide Supervision and Support the assigned Wellness staff
  • Supervises and prepares performance evaluations for assigned staff
  • Provides Clinical services to individuals, families and support groups.
  • In collaboration with Director of FamilyWorks, approves hiring, discipline, promotion, termination and other relevant employment transactions, within defined parameters.
  • In collaboration with management team, ensures Organization policies/procedures for general operations and service delivery. (E.g. personnel, financial, participant eligibility, Standards of Professional Conduct, Ethics, HA's Philosophy of Care, HIPAA etc.)
  • Develops the service plan(s) and service delivery model(s)
  • In collaboration with FamilyWorks' management team, ensures program goals and objectives are met, ensures a quality assurance/audit system for FamilyWorks, in conjunction with HHCS Quality Improvement Plan, and periodically makes appropriate tests of the system.
  • Develops and maintains professional relationships with appropriate groups/organizations, etc., as approved by higher level Manager(s).
  • Monitors program performance outcomes ( MHA, individual counseling and more)
  • Remains current in the field of service through in house trainings and self-initiated avenues, as approved by higher level Manager(s).
  • Provides training to assigned staff in all facets of the position and ensures documentation is completed in a thorough, accurate and timely fashion.
  • Participates productively in Management Team meetings.
  • Conducts internal and external training, as requested.
  • Initiates requests for supervision and interim evaluation/goal setting.
  • Develops long-range plans, periodic assessment reports, success objectives and other significant strategies and/or systems.
  • Ensures the accuracy, substance and timeliness of HHCS reports, projects, schedules and other relevant work products.
  • In conjunction with the FamilyWorks' management team, designs new programs, implements and evaluates new programs in regards to clinical /wellness services and programming according to professional standards of practice and HHCS core competencies.
  • Ensures compliance of FamilyWorks with the standards of the Organization, subsidiary organization CARF.
  • Perform other job-related duties as assigned.

  • Education and/or Experience: Master's degree (M.A) in clinical social work, counseling, clinical psychology, or other related degree; and one or two years of clinical practice experience in either a hospital or community setting providing group and/or individual therapy.
  • Certificates, Licenses, Registrations: Licensed in Clinical Social Work (LCSW) or Licensed Clinical Professional Counselor (LCPC).
  • Language Skills:Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
  • Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent.
  • Reasoning Ability:Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
  • Other Qualifications: A valid driver's license may be required.
  • Computer Skills: Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance.
