DESCRIPTION OF DUTIES/ESSENTIAL FUNCTIONS: Houston's Health Department is seeking a professional individual who will perform the following duties: Performs home visiting in accordance with the Nurse Family Partnership model and guidelines and carries a caseload of up to 25 active clients. Assess the physical, emotional, social, and environmental needs of women and their families as they relate to health and life course development. Develops a therapeutic relationship with women and their families that promote problem-solving self-sufficiency. Assists women and their families to establish realistic goals and measurable outcomes for promoting their health and life course development. Provides motivational interviewing to assist women and their families in attaining targeted goals, in areas including prenatal and postpartum care, nutrition, parenting, child health care, family planning, and special health problems.

WORKING CONDITIONS: The position occasionally requires stooping or bending. Occasional very light lifting, such as three or four reams of papers or books (up to 20 pounds or an equivalent weight) may be required.

EDUCATIONAL REQUIREMENTS: Requires graduation from an accredited school of nursing. Bachelor of Science degree in nursing is preferred.

EXPERIENCE REQUIREMENTS: Four years of experience as a practicing registered nurse are required. A Master of Science degree in Nursing and two years of experience in general nursing, including one year of supervisory experience, may be substituted for the experience requirement.

LICENSE REQUIREMENTS: Valid Driver's License and compliance with the City of Houston's policy on driving (AP 2-2). Must be licensed to practice as a registered nurse in the State of Texas.

PREFERENCES: Applicant who speaks, reads and writes English/Spanish preferred.

Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6.


SELECTION/SKILLS TESTS REQUIRED: None. However, the Department may administer a skills assessment test.


If yes, this position is subject to random drug testing and if candidate is promoted into this position, he/she must pass an assigned drug test.


Grants & Special fund positions are dependent upon continued available funds. If funding is no longer available, employee may be laid off or transferred. Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in this classification.
