We're seeking a dedicated and motivated Physical Therapist (PT) to join our growing team and help patients improve their mobility, independence, and overall quality of life.

What You'll Do as a Physical Therapist (PT)

  • Deliver physical therapy services in accordance with physician's plan of care and state PT practice guidelines

  • Assess and evaluate patients' therapeutic, rehabilitative, and functional status

  • Develop and direct individualized therapy programs focused on rehabilitation and recovery

  • Evaluate home environments and recommend safety/therapy modifications

  • Train patients and caregivers in therapeutic appliances and prosthetic device use

  • Provide instruction and support to families/caregivers and collaborate during case conferences

  • Supervise and guide Physical Therapy Assistants (PTAs) per policy and regulations

  • Monitor and report patients' responses to treatment, adjusting plans as needed

  • Prepare clinical and progress summaries, participate in discharge planning, and coordinate with interdisciplinary teams

What We're Looking For

  • Active Physical Therapy license in Indiana and/or Ohio

  • Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program

  • CPR certification and recent TB test required

  • Valid driver's license, reliable transportation, and auto insurance

  • 2+ years of physical therapy or rehab experience preferred (community/home health experience ideal)

  • Strong communication, organizational, and leadership skills

  • Ability to build rapport with patients and handle physical workload
