CHARTER HOSPITAL CONVERSION
STEP 1: COMPLETE YOUR APPLICATION
This requisition has been created for charter associates who are becoming practice associates as a result of a hospital conversion. Please complete all parts of this application. You will have the opportunity to upload your resume to prefill many of the fields on this application. Please note: your resume must be in a .doc, .docx., .txt, or .pdf format. As resume formats vary, you will want to ensure that your information is captured accurately, and edit if necessary. You can also use your LinkedIn profile, if you have one. Once you have filled out all the fields, you must click "Submit," or else your application will not be complete.
STEP 2: YOUR BACKGROUND CHECK
After you have completed your application, you will need to complete a background check. In the next day or two, you will receive an email from our background check vendor, Occuscreen, which comes from [email protected] This email will contain a link to a secured site. You will be asked to enter your information and submit it for a background check. This information is kept strictly confidential, and is only used for your background check.
STEP 3: YOUR NEW HIRE PAPERWORK
You will also receive another email which contains a link to access the electronic version of the new hire paperwork. This is similar to the new hire paperwork you completed when you began, but in an electronic format.
